Once you have completed the contact form below, you will receive our Terms and Conditions, and Billing Policy documents.

You will then have opportunity to submit PDF documents including all the relevant information such as hospital records, RAF 1 form, notice of instruction and importantly an Acknowledgement of Debt (specific for the radiology practice nearest the location you select).

Please do not submit hard copy documents to the office – we work exclusively electronically.
